Documentation checklist

What your progress note must include by code category to be compliant and audit-safe.

Every CPT code has a documentation floor. Missing elements are the most common reason claims get denied or audited. Use this as your note-writing guide before submitting.

EMDR billing map

CPT code guidance mapped to each EMDRIA phase across a full treatment episode.

EMDR treatment doesn't map neatly onto standard therapy billing — the phases vary in length, purpose, and documentation requirements. This map shows which code fits each phase and what your note needs to reflect.
EMDR intensives (extended sessions): When a processing session runs beyond 82 minutes, add 99354 to 90837. Beyond 112 minutes, also add 99355. Document exact start/end times. Verify coverage with each payer before billing extended session codes.
